ATTENTION ALL PARENTS OF 7TH AND 11TH GRADE STUDENTS:
All students entering the 7th grade must have had one Tdap vaccination and one meningococcal (MCV4) vaccination before the first day of school.
Effective July 2021, all students entering or transferring into the 11th grade must have proof of a meningococcal conjugate booster (MCV4) shot, unless their first dose was received on or after their 16th birthday. This will be required before the first day of school.
These requirements are in addition to the existing school vaccination requirements.
Please schedule an appointment with your child’s doctor or take your child to the local health department to have these required vaccinations.
Please provide the school with the appropriate documentation that your child’s vaccinations are up to date (FORM 3231).
